1. Program Description

Peak Performance Shockwave Therapy ("we," "us," or "our") operates an SMS/text messaging program to communicate with patients and prospective patients. By opting in, you may receive text messages related to appointment reminders, booking confirmations, promotional offers (including our 50% off introductory offer), health tips, and other communications related to our shockwave therapy services located at 1125 NE 125th St, Suite 301, North Miami, FL 33161.

2. How to Opt In

You may opt in to receive SMS messages from us by:

• Providing your mobile number on our online booking or contact form and checking the SMS consent checkbox.
• Verbally consenting at our clinic location and providing your mobile number to a staff member.
• Texting a designated keyword to our SMS number as instructed in our marketing materials.

By opting in, you confirm that you are the account holder or have the account holder's permission to enroll the mobile number, and that you are 13 years of age or older.

3. Message Frequency & Rates

Message frequency varies based on your interactions with us. You may receive up to 4 messages per month, though this may vary. Message and data rates may apply. Charges are billed by and payable to your mobile service provider. Peak Performance Shockwave Therapy is not responsible for any charges imposed by your carrier.

4. How to Opt Out

You may opt out of receiving SMS messages at any time by:

• Replying STOP to any SMS message you receive from us. You will receive one final confirmation message and no further messages will be sent unless you re-opt in.
• Emailing us at Info@peakperformanceshockwavetherapy.com and requesting removal from our SMS list.

Your mobile number and SMS consent will not be sold, rented, or shared with third parties for their own marketing purposes. Your information is handled in accordance with our Privacy Policy.

8. No Medical Advice via SMS
